Cityblock is a value-based provider that partners with health insurance companies to provide care that is both comprehensive and personal. We collaborate with community partners and local healthcare providers to ensure that our members get the care they deserve, when and where they need it.
Our mission is to radically improve the health of marginalized communities at scale.
Our vision is to be the leading nationwide provider of tech-enabled value-based healthcare for individuals with complex needs. Our particular focus is on Medicaid beneficiaries, and those who are dually eligible for Medicare and Medicaid.
Company History
Cityblock Health is the first tech-driven provider for communities with complex needs—bringing better care to where it’s needed most, block by block. Founded in 2017 on the premise that “health is local” and based in Brooklyn, we are backed by Alphabet’s Sidewalk Labs along with some of the top healthcare investors in the country.
